FB HORN

The Faith-Based Health Outreach and Referral Network (F. B. HORN) is a partnership between faith leaders and some of the largest health care information, education and advocacy groups in the country. The resulting collaboration, F. B. HORN will impact alarmingly high rates of disease and infection by sponsoring health events, workshops and seminars at member churches, organizations and other sites across the greater Chicagoland area.

F. B. HORN can help your church or organization by:

* Building strong collaborations and technical capacity within area churches

* Conducting health care and community needs assessments within the targeted community areas to examine and fully understand the most urgent community needs

* In conjunction with professionals from various disciplines, developing a Community Health and Wellness plan that addresses these identified needs

* Conducting focused, goal oriented programming through HORN affiliated churches, social and fraternal organizations and selected residential facilities

* Developing an internal mechanism to monitor outcomes and impact on issues identified in the initial needs assessment
